Call for CPhA Awards Nominations

The California Pharmacists Association’s Awards Ceremony is presented each year at the Western Pharmacy Exchange and is dedicated to honoring individual pharmacists who have dedicated their lives to the profession of pharmacy in the State of California. Pharmacists, technicians, students of pharmacy and CPhA Local Association Chapters are recognized for their support, innovative contributions, leadership, and mentorship to the profession and the community.

  • CPhA Pharmacist of the Year

    Criteria

    This award recognizes a CPhA member pharmacist for outstanding activities in the profession of pharmacy. The recipient must be an individual who has demonstrated leadership at multiple levels of pharmacy. He/she should be an energetic leader who has stimulated others to actively participate in the profession, advocacy, and community affairs related to the practice of pharmacy. The person should be well recognized by his colleagues for his/her professional advancements.

    It would be desirable if the activities or qualifications of the recipient were to result in an especially successful initiative or long-term effort on behalf of the profession. It should be noted that this highest award of the Association is not for a “one-time flash” or to be used as a retirement recognition. The person’s continued activities and efforts are essential.

    • A letter from the nominator to address the criteria and provide specific examples of how the candidate has made significant, influential and sustained contributions to the pharmacy practice in California.
    • Current full curriculum vitae or detailed resume of the nominee.

  • CPhA Excellence in Innovation

    Criteria

    This award recognizes a CPhA member pharmacist that is directly involved in a project or practice of pharmacy that has significantly improved patient care, had a significant impact on the profession of pharmacy, added to the scope of pharmacy practice, created a new type of pharmacy specialty, introduced a concept that has altered the view of the pharmacist, or invented a new chemical composition, method of delivery, raw material or machinery related to the field of Pharmacy.

    Required

    • A letter from the nominator to address the criteria and provide specific examples of how the candidate has made significant, influential and sustained contributions to the pharmacy practice in California.
    • Current full curriculum vitae or detailed resume of the nominee.
    • A photo and professional biography of the nominee.
    • Up to 4 letters of recommendation

  • Bowl of Hygeia

    Criteria

    This award recognizes a pharmacist for outstanding and continuing activity in the community outside of the profession. The recipient should be a person who has demonstrated how pharmacists can elevate themselves and their profession in the eyes of the public through participation in community civic programs or participation in government activities at the local or state level. The person must be a CPhA member pharmacist and have contributed to the advancement of the profession through his/her activities.

  • CPhA New Practitioner of the Year

    Criteria

    This award recognizes the outstanding new practitioner who has demonstrated leadership at multiple levels of pharmacy. He/she should be an energetic leader who has stimulated others to actively participate in the profession, advocacy, and/or community affairs related to the practice of pharmacy. The person should be well recognized by his colleagues for his/her professional advancements. The pharmacist must be licensed, a member of CPhA, and graduated less than 10 years prior.

  • CPhA Student Pharmacist of the Year

    Criteria

    Any student enrolled in a California School of Pharmacy who is a current member of CPhA is eligible to be nominated for this award. The person nominated should be active in the student chapter, active in their local CPhA chapter and demonstrate a willingness to become involved in the future of the profession as evidenced by involvement in CPhA professional activities. The person nominated should be involved in community projects, attending CPhA educational events or other programs designed to bring credit and public acceptance to the profession. Additionally, the student should be involved in advancing his/her school academically and professionally. Each California School of Pharmacy is requested to submit nominations for students that meet the above qualifications.

    Required

    • A letter from the nominator to address the criteria and provide specific examples of how the candidate has made significant, influential and sustained contributions to the pharmacy practice in California.
    • Current full curriculum vitae or detailed resume of the nominee.
    • Up to 4 letters of recommendation
    • One of the letters should be from a professor or advisor of the student chapter

    Criteria

    This award recognizes a CPhA member Pharmacy Technician for outstanding activities in the profession of pharmacy. The individual demonstrates exceptional capabilities, raising the standard of excellence within practice.

    Criteria

    This award recognize any individual who has shown outstanding devotion and service to the profession of pharmacy in California. The nominee should have contributed to, supported, and/or advanced the profession of pharmacy; and should have participated in professional, educational, political and/or community affairs related to pharmacy practice. The nominee need not be a member of CPhA nor a pharmacist.

  • CPhA Chapters of Excellence

    Criteria

    The Chapter of Excellence (COE) Award is a designation of achievement earned by those local chapters meeting the criteria.The criteria for the Chapter of Excellence Award is based upon activities that the local association participates in as a sponsor or includes at least three members as volunteers. The activity may be philanthropic and/or disease state related. An activity may be counted one time per category. For example, if a local is performing both hypertension and diabetes management health screenings at one health fair, it may not be used as a health fair participation and a diabetes screening. In order to earn a Chapter of Excellence award, each chapter will have to earn a total of 100 points. Minimum point values would have to be earned in each listed category.

  • California Pharmacy Hall of Fame

    Criteria

    The California Pharmacy Hall of Fame recognizes individuals who are, or have been, an inspiration to the practice of pharmacy in California, as exemplified by, but not limited to, the following: a long and distinguished history of service, achievement in several arenas, character, innovation, trend setting and altruism. Any individual who has been an inspiration to the practice of pharmacy in California is eligible for nomination. The nominee need not be a CPhA member or a pharmacist. The nominator need not be a CPhA member. Self-nominations are not accepted.

  • Steven C. Feldman Compounding Award

    Criteria

    This award recognizes an individual who is a leader in California pharmacy by virtue of his or her activities to advance the practice of compounding. This must be illustrated in multiple ways, examples including: being a Board member of a pharmacy compounding organization, testifying in front of the Board of Pharmacy, engaging in Legislative Advocacy, giving lectures on compounding, publishing articles on compounding and conducting public education/outreach.  The recipient should be someone who has made significant contributions in compounding and should be an individual who is well respected among his/her compounding colleagues as an individual who advances the profession.

  • Designated Fellow of CPhA

    Become a Designated Fellow of CPhA (FCPhA)

    Being recognized as a fellow of the California Pharmacists Association (CPhA) is an honor. It signifies that you have made significant contributions to CPhA and to the profession of pharmacy that go far beyond regular pharmacy practice.

    The CPhA Board of Trustees has adopted a set of criteria that CPhA members must meet or exceed to be considered eligible to earn the designation of Fellow of the California Pharmacists Association (FCPhA). Selected Fellows will be honored at CPhA’s annual meeting, Western Pharmacy Exchange.

    To become eligible as a Fellow in CPhA, the candidate must have been a licensed California pharmacist and an active member of CPhA for at least seven years. Membership or activities performed while a student pharmacist are not considered for eligibility. Activities during postgraduate residency training may be included. Qualified Fellows should demonstrate sustained contributions and excellence in pharmacy practice, professional leadership, teaching, professional advocacy and community engagement. FCPhA recognition serves as a highly visible way to promote public awareness of pharmacists who have distinguished themselves within CPhA and the profession.

    All application materials must be typed and submitted electronically.  Applicants should submit a curriculum vitae or resume, however, the application form must be fully completed without statements such as “refer to CV”.

    Required Materials

    1. Completed application (submitted electronically)
    2. Curriculum vitae/résumé
    3. A photo and professional biography of the nominee.
    4. Letter of nomination may be submitted by any current CPhA member pharmacist. Self-nominations are acceptable.
    5. Letters of recommendation: minimum of 2; maximum of 5.  Letters should address several of the criteria listed above.* Letters of recommendation are welcome from member pharmacists, technicians or student pharmacists.

    All application materials and letters of recommendation should be received electronically by Friday, January 31, 2020.

  • CPhA-ASP Innovative Chapter of the Year

    Criteria

    This award recognizes California’s most innovative CPhA-Academy of Student Pharmacist (ASP) chapter embodying CPhA’s core values of visionary leadership, innovation, diversity of thought and opportunity, and entrepreneurship. Interested chapters must be active and affiliated with CPhA. The chapter must submit the constitution, bylaws, goals, and annual objectives, as well as an essay highlighting the chapter’s innovative efforts.
