Call for CPhA Awards Nominations
The California Pharmacists Association’s Awards Ceremony is presented each year at the Western Pharmacy Exchange and is dedicated to honoring individual pharmacists who have dedicated their lives to the profession of pharmacy in the State of California. Pharmacists, technicians, students of pharmacy and CPhA Local Association Chapters are recognized for their support, innovative contributions, leadership and mentorship to the profession and the community.

- Pharmacist of the Year
Criteria
This award recognizes a CPhA member pharmacist for outstanding activities in the profession of pharmacy. The recipient must be an individual who has demonstrated leadership at more than one level of organized pharmacy. He/she should be an energetic leader who has stimulated others to actively participate in professional, political, and/or community affairs related to the practice of pharmacy. The person should be well recognized by his colleagues for his/her professional advancements.
It would be desirable if the activities or qualifications of the recipient were to result in an especially successful project or long-term effort on behalf of the profession. It should be noted that this highest award of the Association is not for a “one time flash” or to be used as a retirement recognition. The person’s continued activities and efforts are essential.

Criteria
This award recognizes a CPhA member pharmacist that is directly involved in a project or practice of pharmacy that has either significantly improved patient care, added to the scope of pharmacy practice, created a new type of pharmacy specialty, introduced a concept that has altered the view of the pharmacist, or invented a new chemical composition, method of delivery, raw material or machinery related to the field of pharmacy.

Criteria
This award recognizes a pharmacist for outstanding and continuing activity in the community outside of the profession. The recipient should be a person who has demonstrated how pharmacists can elevate themselves and their profession in the eyes of the public through participation in community civic programs or participation in government activities at the local or state level. The person must be a CPhA member pharmacist and have contributed to the advancement of the profession through his/her activities.

This award recognizes the most outstanding new practitioner who has demonstrated leadership at more than one level of organized pharmacy. He/she should be an energetic leader who has stimulated others to actively participate in professional, political, and/or community affairs related to the practice of pharmacy. The person should be well recognized by his colleagues for his/her professional advancements. The pharmacist must be licensed, a member of CPhA, and graduated less than 10 years prior.

Criteria
Any student enrolled in a California School of Pharmacy who is a current member of ASP/CPhA is eligible to be nominated for this award. The person nominated should be active in the student chapter and demonstrate a willingness to become involved in the future of the profession as evidenced by involvement in CPhA and APhA professional activities. The person nominated should be involved in community projects or other programs designed to bring credit and public acceptance to the profession. Additionally, the student should be involved in advancing his/her school academically and professionally. Each California School of Pharmacy is requested to submit nominations for students from their graduating class that meet the above qualifications.

Criteria
The Chapter of Excellence (COE) Award is a designation of achievement earned by those local chapters meeting the criteria.The criteria for the Chapter of Excellence Award is based upon activities that the local association participates in as a sponsor or includes at least three members as volunteers. The activity may be philanthropic and/or disease state related. An activity may be counted one time per category. For example, if a local is performing both hypertension and diabetes management health screenings at one health fair, it may not be used as a health faire participation and a diabetes screening. In order to earn a Chapter of Excellence award, each chapter will have to earn a total of 100 points. Minimum point values would have to be earned in each listed category.

Criteria
The California Pharmacy Hall of Fame recognizes individuals who are, or have been, an inspiration to the practice of pharmacy in California, as exemplified by, but not limited to, the following: a long and distinguished history of service, achievement in several arenas, character, innovation, trend setting and altruism. Any individual who has been an inspiration to the practice of pharmacy in California is eligible for nomination. The nominee need not be a CPhA member or a pharmacist. The nominator need not be a CPhA member. Self-nominations are not accepted.

Criteria
This award recognizes a compounder who is a leader in California compounding by virtue of his or her activities to advance the practice of compounding. This must be illustrated in multiple ways, examples of which might be a combination of any of the following: as a member of the Board of ACP, testifying in front of the Board of Pharmacy, advocating to state and/or federal legislators, giving lectures on compounding, published articles on compounding and public education/outreach. The recipient should be someone who has made significant contributions in compounding; and should be an individual who is well-known amonghis/her colleagues for their advancement of the profession. The nomineemust be a member of CPhA’s Academy of Compounding Pharmacists; may be a pharmacist or a technician and can be nominated by any CPhA member.

The CPhA Awards Committee is responsible for selecting all qualified award winners, which is based upon careful review of all qualified applicants. Upon receipt of a nomination package, CPhA staff reviews the package for completeness and then forwards on to the CPhA Awards Committee. If the nomination package is incomplete, a CPhA staff member will follow up with the nominator to obtain the missing information.

Become a Designated Fellow of CPhA (FCPhA)
Being recognized as a fellow of the California Pharmacists Association (CPhA) is an honor. It signifies that you have made significant contributions to CPhA and to the profession of pharmacy that go far beyond regular pharmacy practice.
The CPhA Board of Trustees has adopted a set of criteria that CPhA members must meet or exceed to be considered eligible to earn the designation of Fellow of the California Pharmacists Association (FCPhA). Selected Fellows will be honored at CPhA’s annual meeting, Western Pharmacy Exchange.
All application materials must be typed and submitted electronically. Applicants should submit a curriculum vitae or resume, however, the application form must be fully completed without statements such as “refer to CV”.
Required Materials
- Completed application (typed and submitted electronically)
- Curriculum vitae/Resume
- Letter of nomination may be submitted by any current CPhA member pharmacist. Self-nominations are acceptable.
- Letters of recommendation: minimum of 2; maximum of 5. Letters should address several of the criteria listed above.* Letters of recommendation are welcome from member pharmacists, technicians or student pharmacists.
