The Ambulatory Care SIG promotes legislative advocacy, develops communication among health care professionals, advocates standards of practice and provides input on educational programs and services for its Ambulatory Care SIG members.
The Ambulatory Care and Pharmacy Owner Special Interest Group is requesting their SIG Members to complete this survey. The California Department of Public Health (CDPH) is distributing this survey in an effort to understand the preparedness of California pharmacists to implement best practices such as medication therapy management in the treatment of patients with diabetes, hypertension, and/or high blood cholesterol. Participation of front line pharmacists in this survey will help us identify and create local and state resources that will better support your work specifically among patients with diabetes, hypertension, and/or high blood cholesterol management and treatment.
