- Please complete one form for each new course offering. You may want to print a copy for your file before clicking Submit (this will add a date stamp to your copy to confirm when you submitted the form).
- This form must be submitted at least 15 days in advance of each new course offering. Failure to submit this form in a timely manner may jeopardize the Provider’s accreditation status. All online submissions are date/time stamped.
- Coursework submitted by providers with an “inactive accreditation” status will not be approved.
NOTE: This online process is not set up for multiple CE courses that are offered together (i.e. multi-program and/or multi-day symposium). Thank you!
